The Transplant Nurse Coordinator is a Registered Nurse with specialized experience in stem cell or organ transplantation and donation. Working under the supervision of transplant physicians, this position provides comprehensive coordination through all phases of patient care at Northwestern Memorial Healthcare.
Responsible for planning, directing, providing, and documenting appropriate care for patients in the designated care setting. Assist in coordinating comprehensive care for patients, including the consent process, data collection, and arranging required follow-up visits, referrals, testing, treatment, and long-term follow-up.
Work collaboratively with the interdisciplinary team to coordinate ongoing care focused on cost-effective ways to maximize wellness and enhance patients' lives. Develop, coordinate, and provide education for patients, families, and healthcare team members.
This position requires on-call responsibilities, including weekends and holidays on a rotating schedule. Full-time position working day shift in Chicago, Illinois.
Candidates must be a licensed Registered Nurse with experience in and knowledge of stem cell or organ transplantation. Strong coordination and communication skills are essential for success in this role.
