Heart failure represents a major public health challenge resulting in substantial morbidity and mortality, with patients experiencing high hospitalization rates, extended lengths of stay, and frequent readmissions. The BCVI is committed to improving care delivery for heart failure patients, reducing resource utilization and associated costs, and enhancing outcomes through reduced length of stay and readmission rates.
As the Nurse Coordinator, you will ensure consistent, evidence-based care is delivered to all heart failure patients. You will support comprehensive patient care management from admission through discharge by coordinating with physicians, nursing staff, social work, dietary services, and managed care teams. Under the supervision of the BCVI Operations Manager and/or staff physician, you will provide professional nursing care to heart failure patients. Your responsibilities include assessing diverse patient needs including education and discharge planning, working collaboratively with other caregivers to ensure needs are met, and coordinating follow-up appointments. You will function as a vital conduit between inpatient and outpatient care settings, ensuring seamless transitions and continuity of care for heart failure patients throughout their treatment journey.
Candidates should possess a nursing degree with relevant clinical experience in cardiovascular or heart failure care. Strong coordination, communication, and organizational skills are essential. The ideal candidate demonstrates commitment to evidence-based practice, patient-centered care, and continuous quality improvement in heart failure management.
