The mission of the Colorado Coalition for the Homeless is to work collaboratively toward the prevention of homelessness and the creation of lasting solutions for homeless and at-risk families, children, and individuals throughout Colorado. The Coalition advocates for and provides a continuum of housing and a variety of services to improve the health, well-being, and stability of those it serves.

The Medical Assistant III - Patient Navigator (PN) assists patients in “bridging the gaps” within the healthcare system and decreases the barriers to care by utilizing resources. The PN is responsible for providing case management and navigation services to ensure that patients receive recommended services, keep medical appointments, & obtain needed social and financial support.
NOTE: In order to ensure that our consumers receive the best possible care the candidate chosen to fill this position will be required to complete our internal credentialing and privileging process prior to a start date. The credentialing and privileging timeline is dependent on the selected candidate's submission of documents necessary for clearance. Please note that a start date will be scheduled once credentialing and privileging is complete.

Essential Job Functions
Coordinate PN services in partnership with Stout Street Health Center’s integrated provider and nursing staff. Meet with patients to develop treatment plans and monitor treatment progress. Provide care management to individual patients as assigned. Assist in scheduling appointments for screenings, follow-up on diagnostic services, and appointments with specialists, treatment, and/or surgery. Ensure all required medical notes and diagnostic test results are available at the time of appointment. Help schedule colonoscopies for clients per their provider’s order. Educate scheduled patients on colonoscopy prep. Arrange motel and cab vouchers for those clients who need a place to stay the night prior to the colonoscopy to complete their prep for the procedure. Call the client the day before the procedure to remind them when to arrive, etc. Pick up patients after procedures if necessary. Organize and provide detailed and timely documentation of referrals and their stage of process in the EHR. Track case status, interventions, and outcomes on a regular basis and gather statistics as required for reporting purposes Patient Navigation : Assist with arranging transportation and help clients arrive at scheduled appointments on time. Transport patients as needed and appropriate. Assist in informing clients about results of tests and appointments. Guide clients through the healthcare system, facilitate interaction and communication between the client and health care staff and providers, and advocate for their needs. Help decrease client fear and anxiety by providing relevant and appropriate health information and emotional support. Outreach patients in the community
